รายงานผู้ป่วย 1 ราย: Mucous Membrane Pemphigoid ; Case Report: Mucous Membrane Pemphigoid

มิวคัสเมมเบรน เพมฟิกอยด์ เป็นโรคเรื้อรังที่เกี่ยวข้องกับความผิดปกติของการตอบสนองของระบบภูมิคุ้มกันของร่างกาย ลักษณะของรอยโรคเป็นตุ่มน้ำใส ซึ่งสามารถเกิดได้กับเยื่อบุผิวต่างๆของร่างกาย ได้แก่ ช่องปาก เยื่อบุตา คอหอย หลอดลม หลอดอาหาร อวัยวะเพศและทวารหนัก บทความนี้ได้รายงานผู้ป่วยหญิง 1 ราย อายุ 47 ปี มาพบทันตแพทย์ด้วยอาการมีแผลบริเวณเหงือกด้านบนซ้าย มีลักษณะเป็นแผลตุ่มน้ำใส มีแผลหลุดลอก มีอาการปวดแสบเวลารับประทานอาหารเผ็ดร้อน เกิดขึ้นเป็นเวลานานหลายเดือน ไม่พบอาการที่บริเวณอื่นๆ ของร่างกาย จากการตรวจทางคลินิก และการส่งตรวจชิ้นเนื้อ วินิจฉัยได้ว่า ผู้ป่วยเป็นโรคมิวคัสเมมเบรน เพมฟิกอยด์ ทันตแพทย์ให้การรักษาร่วมกับแพทย์ผู้เชี่ยวชาญด้านโรคผิวหนัง โดยให้ด้วยสเตียรอยด์ชนิดทาเฉพาะที่ ร่วมกับให้สเตียรอยด์ชนิดรับประทาน จากการติดตามผลการรักษาเป็นเวลา 1ปี ผลการรักษาเป็นที่น่าพอใจ และยังไม่กลับมาเป็นซ้ำอีก ; Mucous membrane pemphigoid (MMP) is an autoimmune disease that is characterized by vesiculobullous lesion involved mucous membranes of the oral cavity, conjunctiva, nasopharynx, larynx, esophagus, genitourinary tract, and anus. This was a case report of 47 years old female patient who presented to our dental clinic with characteristic lesions of attached gingiva at left maxillary arch. The clinical appearance was small fluid blister, irregular ulcer and yellowish slough. She had chronic oral soreness when taking spicy food for long time. Other membranes of the body were not affected. By clinical and microscopic examination, this case was diagnosed of having Mucous membrane pemphigoid (MMP). Dentist and dermatologist had cooperative treatment, prescribed topical and systemic steroid. The patient was under observed and 1year follow up. The treatment was satisfied and lesion had no recurrence.
