45mm径ステントグラフト中枢側の大動脈拡大に対するopen stent graftを用いた弓部大動脈全置換術の1例 ; 45mm ケイ ステント グラフト チュウスウガワ ノ ダイドウミャク カクダイ ニ タイスル open stent graft オ モチイタ キュウブ ダイドウミャク ゼンチカンジュツ ノ 1レイ ; Total arch replacement with open stent graft placement for the proximal aortic neck enlargement of 45-mm stent graft: a case report

症例は74歳男性．3年前に施行した左鎖骨下動脈直下 (Zone III)の胸部大動脈ステントグラフト内挿術後の中枢側大動脈の拡大で手術の方針となった．45mm径と大口径のステントグラフトが留置されており, 人工血管の末梢吻合をステントグラフトに直接行うことは, 口径差やステント拡張力の観点から困難と考えられた．胸骨正中切開でアプローチし人工心肺確立の後, 左鎖骨下動脈を閉鎖し, 左総頚動脈直下 (Zone II)の弓部大動脈に末梢吻合口を作成した．そこからopen stent graftを末梢に向けて挿入し, 弓部大動脈全置換術を施行した．術後3日目造影CTで, open stent graft末梢端でのエンドリークは無かった．経過良好で, 術後11日目に独歩退院となった． ; A 74-year-old man underwent surgery for proximal aortic neck dilation following Zone III TEVAR. Direct distal anastomosis of the prosthetic graft to the 45-mm stent graft was challenging because of size mismatch and the radial force of the stent. The operation was performed via median sternotomy under cardiopulmonary bypass. The left subclavian artery was transected at its origin, and the proximal stump was oversewn. The distal anastomotic site was created on the aortic arch at Zone II. An open stent graft was then introduced in an antegrade fashion, deployed, and followed by total arch replacement. Postoperative contrast-enhanced CT on day 3 demonstrated no endoleak at the distal end of the open stent graft. The patient’s postoperative course was uneventful, and he was discharged without any complications on postoperative day 11. ; departmental bulletin paper
