      Hypertension is a significant public health issue, affecting over 1.3 billion people worldwide (Maet al., 2022). In the United States, it stands as the leading cause of heart disease, stroke, and kidney failure (Fong, 2019). Despite the availability of effective treatments, many individuals with hypertension struggle to maintain practitioner-driven health coach program for adult hypertension patients in a rural primary care controlled blood pressure. This project assessed a nurse facility (Hoppe et al., 2023). The program encompassed education, counseling, and support, utilizing a quasi-experimental design in a pre-post project. The intervention group exhibited significant improvements in self-care knowledge, as measured by the SC-HI V3 hypertension self-care model (Dickson et al., 2021). The findings suggest that a nurse practitioner-driven health coach program can effectively enhance self-care knowledge and blood pressure control among adult hypertension patients in rural primary care settings. These results hold implications for preventing and managing hypertension in rural populations.
