    • Abstract:
      Healthcare professionals are obliged to offer patients conversations about existential questions. Advanced careplanning (ACP) focuses on a patient’s end-of-life wishes but is rarely carried out. This study aimed to explorehealthcare personnel’s experiences with participating in ACP within the framework of a person-centred practicein nursing homes. Seven Norwegian healthcare workers took part in individual interviews. The results show thatthe organization of the ACP was of importance for the implementation. Predominantly, it was the next of kin whoparticipated and advocated on behalf of the patient. Only two patients directly participated in the ACP. However,the “patient’s voice” also appeared through brief daily conversations. The ACP documentation did not always reachall healthcare workers who needed it. Further research is needed to explore ways in which people with dementiacan engage in ACP, establish clear documentation routines, and ascertain how ACP can enhance person-centredpractice.
