REVISION SISTEMÁTICA SOBRE PSICOTERAPIAS EFECTIVAS Y/O TRATAMIENTOS COMBINADOS CON PACIENTES CON SEVERIDAD Y COMORBILIDAD

There are currently a variety of effective psychotherapies for most psychopathological disorders. When the diagnosis implies severity and co-morbidity in the psychiatric symptomatology, the indication is combined treatment, medication and psychotherapy. This systematic review study aims to collect scientific articles on effective psychotherapies and/or combined treatments with patients with severity and comorbidity from the perspective of evidence-based practices. For this purpose, the PRISMA declaration principles and the PICOS criterion were applied. In a first search, 95 (N=95) articles were selected, of which 90% of the analysis units came from databases in Europe and the United States. Three other searches were carried out, the first one with the keywords "evidence-based psychotherapies and combined treatments", the second with "psychotherapy and medication" and the third with "evidence-based psychotherapy", of which 53 articles were selected (N= 53), excluding the findings of Europe and USA databases because they yielded too extensive results. It is noticeable the fact that in the more developed countries, there is a significantly greater presence of research studies on the efficacy and effectiveness of combined psychotherapies aimed at specific disorders, including severity and comorbidity, consistent with a greater availability of funding research in those areas. Given the high prevalence and high amount of suffering for patients and their significant others, the importance of disseminating this knowledge to improve the results of psychotherapies with these patients is highlighted
Resumen: Existen actualmente una variedad de psicoterapias efectivas para la mayoría de los trastornos psicopatológicos. Cuando el diagnóstico implica severidad y comorbilidad en la sintomatología psiquiátrica la indicación es tratamiento combinado, medicación y psicoterapia. Este estudio de revisión sistemática tiene como objetivo recolectar artículos científicos sobre las psicoterapias efectivas y/o tratamientos combinados con pacientes con severidad y comorbilidad desde la perspectiva de las prácticas basadas en evidencias. Para ello se aplicó lo formulado en la declaración PRISMA y el criterio PICOS. En una primera búsqueda se seleccionaron 95 (N=95) artículos, el 90 % proveniente de bases de datos de Europa y Estados Unidos. Se realizaron otras 3 búsquedas, la primera con las palabras clave ¨psicoterapias basadas en evidencias y tratamientos combinados¨, la segunda con ¨psicoterapia y medicación¨ y la tercera con ¨psicoterapia basada en evidencias¨, de las cuales se seleccionaron 53 (N=53) artículos, pero se excluyeron las bases Europa y USA porque arrojaron resultados demasiado extensos. En los países más desarrollados, se observa una presencia significativamente mayor de estudios de investigación sobre eficacia y efectividad de las psicoterapias combinadas dirigidos a trastornos específicos, incluyendo severidad y comorbilidad, coherente con una mayor disponibilidad de financiamiento para llevarlos adelante. Dada la alta prevalencia y el elevado monto de sufrimiento que acarrean los pacientes y sus allegados, se destaca la importancia de difundir estos conocimientos para mejorar los resultados de las psicoterapias con estos pacientes.
